Health Requirements

Rockford Lutheran School Health, Dental, Vision Examination & Immunization Requirements for School Attendance 2017-2018

 The following items are required of all students attending school in the State of Illinois:

HEALTH EXAMS – A health exam must be completed no earlier than a year (365 days) prior to the start of attendance for students entering…

  • Preschool
  • Kindergarten
  • 6th grade
  • 9th grade
  • and students entering an Illinois school from out-of-state or out-of-country for the first time at any grade level
  • LEAD SCREENINGLead screening is a required part of the health examination for children age 6 or younger prior to admission to kindergarten
  • The required health exam/immunizations and lead screening may be obtained at the doctor’s office of your choice (chiropractic not accepted)
  • The Certificate of Child Health Examination Form labeled “11/15” must be used by students who need to provide proof of the physical exam for school year 2017-18 (forms available at your doctor’s office) http://dph.illinois.gov/sites/default/files/forms/certificate-ofchild-health-examination-03032017.pdf 
  • State law requires that students without the above exam and/or vaccinations are EXCLUDED from school attendance unless certain exceptions are met by October 15, 2017.  There are only two valid exemptions in Illinois to the above requirement---an exemption for a  medical reason ( written and signed by a health care provider), or for a religious reason (refer to http://www.dph.illinois.gov/sites/default/files/forms/religious-exemption-form-081815-040816.pdf  for more information)

DENTAL EXAMS- All students in Kindergarten, 2nd and 6th grades are required to show proof of having an oral health exam  prior to May 15, 2018

  • Examinations must be performed by a licensed dentist, and he/she must sign the Proof of School Dental Examination Form (available at your dentist’s office) http://www.dph.illinois.gov/sites/default/files/forms/dentalexamproof10_0.pdf
  • School dental examinations must have been performed within the 18 months prior to the May 15, 2018 deadline
  • Failure to submit proof of exam may result in a hold of the student’s report card

EYE EXAMS – All students enrolling in Kindergarten and/or any student enrolling for the first time are required to have an eye exam before October 15, 2017

  • Eye examinations must be performed within one year prior to the first day of the school year
