Health Requirements

The following are the Health, Dental & Vision Examination & Immunization Requirements for all students attending school in the State of Illinois:

HEALTH EXAMS – A student must have a physical health examination within one year prior to entering-

  • Preschool, Kindergarten, 6th and 9th grades, and students transferring into Illinois from outside of the State or Country
  • New students to RLS, regardless of age/grade, must show proof of exam/ immunizations within 30 days of registration
  • ImmunizationsVaccination against a number of communicable diseases at specified intervals is required (see “Minimum Immunization Requirements for Those Entering a Child Care Facility or School in Illinois, Fall-2015”).  Note that beginning in August, 2015, the meningitis vaccine is required for entry  into 6th & 12th grades
  • Lead ScreeningLead screening is a required part of the health examination for children age 6 or younger prior to admission to kindergarten
  • The required health exam/immunizations and lead screening may be obtained at the doctor’s office/clinic of your choice (chiropractic not accepted) and must be completed on form IL444-4737 (R-01-12) – (forms are available at your doctor’s office/clinic)
  • State law requires that students without the above exam and/or vaccinations are excluded from school attendance unless certain exceptions are met.  The only valid exemptions in Illinois to the above requirements are those based on medical reason – written by a health care provider, or religious reason – written by a parent or guardian and submitted to the student’s health file
  • IMPORTANT- The State of Illinois has established an exclusion date of October 15th to meet these requirements for health examinations and immunizations

DENTAL EXAMS - All students in Kindergarten, 2nd and 6th grades are required to have an oral health exam prior to May 15th of the school year.

  • Examinations must be performed by a licensed dentist, and he/she must sign the Proof of School Dental Examination Form (available at your dentist’s office)
  • School dental examinations must have been performed within the 18 months prior to the May 15th deadline
  • Failure to submit proof of exam may result in a hold of the student’s report card

EYE EXAMS – All students enrolling in kindergarten and/or any student enrolling for the first time in a public, private, or parochial school are required to have an eye exam before October 15th of the school year.

  • Exams must be performed by a licensed optometrist or medical doctor who performs eye exams.  He/she shall complete and sign the Eye Examination Report Form (available at your eye doctor’s office)
  • Eye examinations must be performed within one year prior to the first day of the school year
  • Failure to submit proof of exam may result in a hold of the student’s report card

Attention Parents of students entering 6th & 12th grades in Fall, 2015–A new vaccine, the meningitis vaccine, is required for entry into grades six and twelve beginning in August, 2015.  This vaccine protects against a severe form of bacterial meningitis, as well as other diseases caused by this bacteria.  Please make plans to assure that your student is protected against meningitis and ready to start school in the fall.
