Rockford Lutheran School Health Requirements

All of the following medical requirements must be completed in order to attend school in the state of Illinois.

  • Physical Examination: The Illinois School Code requires that each child present proof of having a health examination in accordance with the laws and rules of the Department of Public Health at each of the following times:
    • Within one year prior to entering pre-school, kindergarten, sixth grade, ninth grade and new students entering the school district.
    • Upon transferring from an out-of-state school, irrespective of grade exams are due 30 days from enrollment. If an out of state form is used, it must contain all he same health information as the Illinois form. Check with the school nurse.
    • A licensed medical provider shall conduct the health examination and record it on the State of Illinois form; chiropractic examinations are not acceptable. Failure to submit proof of a current health examination will result in exclusion from school.
  • Dental examination: All Illinois children in pre-school, kindergarten, second and sixth grade will be required to have an oral examination. Each child shall present proof of examination by a dentist prior to May 15 of the school year.
  • Eye Examination: Shall be administered by a doctor licensed to perform eye examinations, or optometrist in accordance with Public Act 095-0671. Eye exams include history, visual acuity, subjective refraction to the best visual acuity near and far, internal and external exam and glaucoma evaluation and any other necessary test. Students enrolling in kindergarten and any student enrolling for the first time in an Illinois school shall have an eye examination.
  • Immunizations: The Illinois Department of Public Health requires proof of the following immunizations at the same intervals as the Physical Exam. Failure to submit immunizations will result in exclusion from school.
